When incidents or near misses occur, it is incumbent that we learn from them to create a safer environment and better processes. Whether they take place within the College, elsewhere on Campus, or around the world, they serve to educate and inform us. While we seek to avoid them, incidents and near misses can serve as topics of reflection and discussion within your department or research group.
